I think the only way you are going to be able to do this is by using the ArcGIS Pro SDK for .Net which supports the creation of forms:

If you want to do this as a web app then I suspect you will have to use the ArcGIS Maps SDK for JavaScript (More than happy to be corrected on this point from anybody else who might read this..):
