Coded values displaying incorrectly in Edit Widget

Two coded values are displaying their numerical codes rather than the label in the Edit widget. The symbology is correctly displaying for these layers.

The labels are properly displaying in the webmap. They are also displaying properly in the popup, the dropdown menu of the edit widget and the Table widget.

Could this be an issue in the SmartForm? I do not have access the the form for this feature currently.

It was an error in the feature layer. The problem was resolved after re-publishing the feature service.

This is happening to us with a feature service with contingent values in Enterprise 11.1.
