Issues annotating an image in Survey123

I uploaded a very basic .jpg (a black rectangle to fill in for a soil profile) the image shows up but when I go to annotate it, the image doesn't show up in the annotation window.

I just want to be able to draw horizontal lines across the rectangle and list the Munsell colors in each layer. 

It doesn't show up in Survey123 Connect or on the app in my iPad.

Attached is the .xls for the form.

The form is fine. Can you share that the image you are using and I will try to replicate.

Attached below. It's just a simple .jpg that I made in paint.

All you need to do is go to the file and copy the filename (including extention: stratum_profile.jpg) and then over paste it into the media::image column. I know that you have done this already - but for some reason Connect is not happy with the contents of the cell. Over pasting the value fixed to survey for me.
