Once only display of a note with relevance to 4 questions

I want to display a note once, that is relevant to 4 questions. I don't want the note to display multiple times if 'choice 4' is selected for more than 1 of the questions.

When I have the relevant condition on 1 question it displays correctly:

If I combine the conditions with Or statements it returns an error message:

Two questions are in one group and the other two questions are in a second group.

Can someone suggest a correct syntax to achieve this?

I found the syntax required by actually reading (rather than skimming it) the Types worksheet of the survey spreadsheet: 'Or' changed to 'or' solved the problem!
