Pulldata function working in Connect but not working when published to Enterprise Portal

I have a very simple survey to pull data of Lat and Long coordinates from a media list (CSV). It works as intended when testing in S123 Connect app but not in browser when testing in our enterprise portal. Any thoughts on what I am doing wrong here? Thanks in advance for any help!
