To use the world traffic service in a map that is to be shown to people without an ArcGIS developer/online account, is there a $ cost?

You can use the Traffic Service in your application as long as you are able to provide proper authentication tied to an ArcGIS Developer or ArcGIS Online account. You can review the authentication methods here https://developers.arcgis.com/documentation/mapping-apis-and-services/security/.

To OP's point, is there anyway to use the World Traffic Service without having users authenticate through ArcGIS Online? (i.e. if we published a public map with the World Traffic Service, could users without an ArcGIS Online account still access the World Traffic Service?) If not, would we have to access a monetized service?

This is an article by Eric Rodenberg that goes through how to setup an API to an ArcGIS online service through enterprise. This may help with your issue https://community.esri.com/t5/roads-and-highways-blog/adding-esri-s-world-traffic-service-to-a-web/b...
