Unidentified Error Failed to execute (ReportCacheStatus)

ArcGIS Server, on Windows Server 2003 SP3 x64, I've published a service from Arcmap. The data appears to have uploaded to the server okay, but pointing a browser at the map service shows a blank page (excepting the "ArcGIS JavaScript API" header and the zoom control).

When using ArcGIS Server Manager to look at the Cache Status I'm told "Error executing tool.: ERROR 999999: Error executing function. Unidentified Error Failed to execute (ReportCacheStatus)."

Under Manage Services >> {selection} >> Edit >> Caching the following are enabled:

Use tiles from cache (e:\arcgissserver\directories\arcgiscache)
   - Dynamically from the data  tried also, same error)
  
Create tiles on demand (have tried disabled as well)
Allow clients to cache locally

How do I troubleshoot this?

Arcmap "Manage Server Cache Tiles" geoprocessing tool on the same service also fails with Error 999999:

Hi Jade, 

Sorry I don't remember what happened here. I think we just gave up and moved on to other things. We haven't needed to use or work with tile caches for a long time.
