ArcGIS program/toolbar used to create this?

Trying to generate a map for my fire department using the same parameters as this attached map, however FireRunBook, isn't an exact match. I'd like to generate a street index with address points, and have an area map page, then the detailed map pages following.  Any ideas on tool/perhaps is it not ArcGIS? Any guidance would be helpful.

This doesn't look like an ArcMap creation judging by the way the text boxes run off the page.

You can, however, write a script that will do this with Python and the arcpy.mapping module. This would allow for the best customization, but you might get by with the standard 'data driven pages' tools.

If you're headed to the UC, go to some of the arcpy.mapping or data driven pages sessions, or look for the new Python GIS Tutorial  programming book that has examples of how to do this.
